  • Providers will be asked to submit:
    • Collaboration Agreement with Medicaid
    • Supervisor Qualifications
    • One supervisor may have a max of 20 CHWs
    • State CHW Certificates
    • MCO contracts as defined

Planning Resources:

Classes are offered 4 times per year or by group class. For group classes up to 30, we can put an instruction team together for $4,500 or $300 per student. Additional costs apply to mentorship and field placement support. Donations, course bookings, and grants to the CHW Training Fund:

  • Ensure that all communities have access to this training
  • Overcome training barriers
  • Hire public health leaders to instruct classes
  • Provide mentorship over the 8-week course
  • Keep this training affordable to incoming CHWs
  • Limit or eliminate class wait times
